One of the most common things I hear in clinic is:

“My period arrives almost as soon as I’ve ovulated.”

Or:

“I spot for days before my period starts.”

Many women assume this is just how their cycle is.

But often it’s a sign that the luteal phase - the second half of the cycle - isn’t being sustained for as long as it should be.

And many women have never even heard of the luteal phase.

Yet it’s one of the most important phases of your cycle.

It’s the time between ovulation and your period, when progesterone rises and your body shifts into the second half of the menstrual cycle.

When that phase is shorter than it should be, it can show up in different ways.

For some women, it’s difficulty conceiving.

Pre-period spotting is one of the most commonly dismissed symptoms I see in my clinic.

A little brown spotting.
Sometimes pink.
A few days before your period properly arrives.

And so many women are told it’s “normal”.

But in clinic, I pay attention to it.

Because spotting before a period is often a sign that progesterone isn’t being sustained well enough in the second half of the cycle.

Progesterone is what helps maintain the uterine lining after ovulation.

So when levels begin to drop too early, the lining can start breaking down ahead of time and that’s often what you’re seeing as spotting.

This is one of the reasons I focus so much on the luteal phase with my fertility patients.

Low AMH has become one of the most emotionally loaded fertility results women receive.

I see it all the time in my clinic - women arriving already convinced their fertility has been decided because of one number on a blood test.

And whilst AMH can absolutely give useful information, it was never designed to predict whether you can or cannot get pregnant.

Originally, it was introduced to help estimate how the ovaries might respond to stimulation medication during IVF. In other words, it gives an indication of possible follicle response - not the quality of the eggs developing inside those follicles.

That distinction matters far more than most women realise.

Because egg quality is influenced over roughly three months, and during that time things like blood sugar balance, nutrient availability, inflammation, stress physiology and overall health all shape the environment those eggs are developing in.

This is why I never look at AMH in isolation.

I look at the full picture:
your cycle, ovulation patterns, symptoms, stress levels, sleep, lifestyle and overall hormonal health too.

Low AMH may tell us something important.
But it is not the full story.

Low progesterone is something I’m often asked about when someone is trying to conceive.

But what’s interesting is that it doesn’t always show up in obvious ways.

Sometimes the signs are quite subtle, and easy to overlook.

You might notice...
🩸 your period arrives a little sooner than expected after ovulation, so the second half of your cycle feels shorter than it should.
🩸 there’s spotting in the days leading up to your period, even when everything else seems fairly regular
🩸 some women describe feeling quite different after ovulation too - lower mood, more anxious or not sleeping as well - without always linking it back to hormones.

In some cases, it can also show up as difficulty maintaining an early pregnancy, as progesterone plays an important role in supporting the uterine lining at that stage.

And often, when we look a little closer, we see that ovulation itself isn’t as strong or consistent as it could be, which means progesterone doesn’t rise as well afterwards.

This is something I talk about a lot in my clinic.

Progesterone is important, but it usually reflects what’s happened earlier in the cycle.

So instead of just trying to increase progesterone, we look at...

I’d focus on the few things that actually make a difference.
